Blue Shield of California Custom PPO 500 90/70

Under the Blue Shield of CA PPO, you may choose your own physician each time you receive care. A non-Blue Shield PPO physician will be paid at 70/100% of eligible charges according to customary and reasonable rates. A Blue Shield of CA PPO physician will be paid based on the negotiated rate at 90%. Therefore, your out-of-pocket cost will be lower when you use a Blue Shield of CA PPO provider.

This Plan uses a specific network of Health Care Providers, called the Access+ HMO provider network. Medical Groups, Independent Practice Associations (IPAs), and Physicians in this network are called Participating Providers. You must select a Primary Care Physician from this network to provide your primary care and help you access services, but there are some exceptions.

Delta Dental pays 70% of approved fees during the first year you are eligible. This percentage will increase 10% each year (to a maximum of 100%) for each enrollee, provided that person visits the dentist at least once during the year.

A part-time faculty member shall be eligible for the same District sponsored healthcare coverage provided to full-time faculty members so long as the part-time faculty member meets one of the following criteria as defined under Education Code Section 87861 et seq. The District and Guild understand the eligibility requirements to be the following:

  • Has a teaching assignment at GCC that is greater than or equal to 40% of the minimum full-time teaching assignment at that district; or

  • Has a teaching assignment as a part-time faculty member at GCC and one or more California community college districts which in total is equal to or is greater than 40% of a full-time teaching assignment.

  • The employee may also meet this 40% load or assignment requirement for the Spring semester, if their annual load is at least 80% by combining their Fall and Spring assignments from the current calendar year and they have an assignment at Glendale college in both semesters.

You must enroll in benefits within 31 days of your start date.

As a full-time employee, your benefits become active at the start of the month following your hire date. For instance, if your employment begins on April 2nd, your benefits will commence on May 1st. However, if you start on April 1st, your benefits will be effective immediately.

Glendale Community College pays 100% of the monthly premiums for all full-time* employees and their eligible dependents. This includes medical, dental, vision, basic life, and basic accidental death & dismemberment insurance.

*Classified employees working part-time at least 20 hours a week but less than 30 hours, the District will pay the entire monthly premiums for employee-only coverage.

Eligible dependent children are covered until the end of the month in which they turn 26 years old for Blue Shield and Kaiser. Dental and vision coverage is until the end of the month in which they turn 24 years old. Once coverage ends, COBRA coverage will be offered to the dropped dependent and mailed to the employee's address on file.

For disabled dependents, coverage will continue with the submission of a doctor's note.

Ineligible dependents will be automatically removed from the plan within 31 days after their 26th birthday. Coverage will end at the conclusion of the month they turn 26. However, they may continue coverage through COBRA for an additional 36 months.

To add a spouse to the health/dental/vision plans, you must complete change forms within 31 days of marriage. You will be asked to submit supporting documents, such as a marriage license or a court document.

No, do not wait till you receive the Social Security Card or birth certificate. You can submit a copy of the hospital birth certificate. Please notify benefits as soon as your child is born. You have 31 days from the baby’s birth to add him or her to your benefit coverage.

Yes. You will need to submit a copy of your domestic partnership certificate from the State of California.

Examples of qualifying events include: birth, legal adoption, marriage, divorce, legal separation, dependent child reaches age limit, spouse gains/loses employment or eligibility with current employer, death of spouse or dependent child, change in residence that changes eligibility for coverage, court-ordered changes.

Open enrollment is normally around mid-November through mid-December for full-time employees, with changes taking effect January 1st of the following year.

Yes, as long as you meet the requirements outlined in the Adjunct Benefits section above.

Yes. As long as you meet the requirements for adjunct faculty to enroll, you can sign up for coverage so there will not be a lapse in coverage for you. Please note that we will need verification of your loss of coverage through your other District.

No. There is still a 40% load equivalent for non-teaching adjuncts.

As long as your annual load is at least 80%, i.e. 60% Fall and 20% Spring, you will still qualify for coverage.

If your annual load is not at least 80%, you will lose your eligibility and coverage will terminate at the end of the month.

No. They cannot be concurrently covered. They will need to drop Part B and the supplemental coverage.

Yes. They will lose any related full-timer benefits associated with their previous position.
